• Record of equipment calibrations or recalibrations and any actions taken as a <br /> result of recalibration. <br /> • Site visits by others. <br /> • Identification of construction problems and their solution or disposition. <br /> Inspection Data Sheets <br /> All field observations and field testing will be recorded on an inspection data <br /> sheet which in combination with the technician's daily reports will be used to <br /> formulate the daily summary report. All field testing will follow ASTM standard <br /> test procedures in accordance with the CQA Plan in the specifications. <br /> Observations in the field may take the form of the ASTM notes, charts, drawings <br /> or sketches, photographs, or any combination of the above. The inspection data <br /> sheets will contain the following minimum information: <br /> • Description and title of the inspection activity. <br /> • Time the activity was performed. <br /> ® Location of the inspection activity. <br /> • Weather conditions. <br /> • Site conditions. <br /> • Standard test method used or type of inspection activity. <br /> • Test equipment used. <br /> ® Record of observation and/or test data, with all calculations completed and <br /> checked. <br /> • Comparison of test results and observations with specification requirements. <br /> • Names and titles of all persons involved in the inspection activity. <br /> • Record of any material or workmanship that does not meet specified designs <br /> and all corrective action measures and results. <br /> Monthly Construction Summaries <br /> Monthly construction summaries will be prepared by the CQA Manager and <br /> include the following items: <br /> • Inspection dates. <br /> • Time spent on the site. <br /> Forward Landfill JTD .5-30 <br /> L\Allied\2000.193\Reports\ITD-PC PCMP702:Sec-5.0:5/24/02 <br /> Revision 1:07/T6/02 <br /> BRYAN A.STIRRAT&ASSOCIATES <br />
