Mr. Kevin ChiaPe llo • • <br />1/16/2004 <br />Page 2 <br />Forward Inc. is hereby notified that Board staff is proposing to include Forward Landfill Inc. <br />(SWIS #39 -AA -0015) on the Inventory pursuant to Public Resources Code Section 44104 for repeat <br />violations of Title 27, CCR, Section 20919.5 — Explosive Gases Control. Forward Landfill Inc. will <br />have 90 days to correct this violation from the notice date in order to avoid being placed on the <br />Inventory. <br />The local enforcement agency will continue to be responsible for inspecting Forward Landfill Inc. on a <br />monthly basis and providing written documentation (inspection reports) regarding the compliance status <br />with respect to the state minimum standard cited above. If the violation is corrected to the satisfaction <br />of the local enforcement agency within 90 days of receipt of this notice, Forward Landfill Inc. will not <br />be included on the Inventory.
