THERFORE, PURSUANT TO PUBLIC RESOURCES CODE DIVISION 30 AND <br />CALIFORNIA CODE 1 DIVISION 7, YOU <br />HEREBY ORDERED O <br />1. Continue routine explosive gas monitoring program. The monitoring program <br />shall include, but not limited to, the evaluation of the existing gas monitoring <br />probes and the concentration of landfill gas in the ambient air at ground surface. <br />The minimum frequency of monitoring shall be monthly. <br />2. Submit gas monitoring reports to this LEA in accordance with the following <br />schedule: <br />Monitoring Period <br />January -March <br />April -June <br />July -September <br />October -December <br />Reporting Deadline <br />April 30 <br />July 30 <br />October 30 <br />January 30 <br />3. Evaluate the effectiveness of phase 1 and 2 gas extraction wells of Forward Inc. <br />corrective action plan. Submit the detailed report. including site map showing the <br />locations of all gas extraction wells, to this LEA by June 30, 2004. <br />4. The LEA has agreed to allow the installation of Phase 3 gas extraction wells of <br />Forward Inc. corrective action plan, provided that Phase 1 and 2 gas wells have <br />been installed and put in operation and have had no effect on the gas levels in the <br />perimeter monitoring wells. Prior to installing the Phase 2 gas extraction wells the <br />operator must submit the phase 3 action plan to this LEA for approval prior to <br />commencing the installation of phase 3 gas extraction wells of the corrective <br />action plan. The phase 3 action plan shall include, but not limited to, the <br />proposed date of installation and destruction, the destruction plan, and any <br />changes to the corrective action plan. All Phase 3 gas extraction wells shall be <br />decommissioned within 120 days of installation and destroyed with in 180 of <br />construction. <br />5. Evaluate the effectiveness of all 3 phases of gas extraction wells. Submit the <br />report, including site map showing the locations of all gas extraction wells, to this <br />LEA by October 30, 2004. <br />6. If all 3 phases of gas extraction wells fail to reduce the concentration of explosive <br />gas at the boundaries to less than 5%, submit a new corrective action plan to this <br />LEA by November 30, 2004. The new corrective action plan shall include, but <br />not limited to, study of gas migration at the above-mentioned facility and the <br />proposed action and schedule for compliance. <br />Amended Notice and Order 03-01 Paee 5 of 7 <br />May 10, 2004 <br />Forward Landfill <br />
