• appropriate level of analysis to meet California Environmental Quality Act(CEQA) in <br /> order to revise the permit. The existing CEQA for the facility does not address the <br /> procedure of taking waste above the permitted tonnage. The meeting ended with Forward <br /> management saying that they were going to look in to it. <br /> On June 10, 2005,Robert McClellon, received an email from Heidi Melander a consultant <br /> for the City of Stockton. Ms. Melander was looking to obtain a copy of the letter that for <br /> submitted regarding the processing of the green waste to remove the food waste. I phoned <br /> Ms. Melander and informed her that the EHD was required Forward Inc. to revise the <br /> initial document to provide more detail and to put it on Forward Inc. letterhead. I faxed a <br /> copy to her and forwarded an email from Kevin Basso that had a word document <br /> explaining how the green waste was going to be processed to remove the food waste (see <br /> exhibit rr). <br /> On June 13, 2005 Robert McClellon received an email from Heidi Melander, consultant <br /> for the City of Stockton. Ms. Melander had reviewed the fax regarding the green waste <br /> program and had concerns. I responded to her email that day with comments regarding <br /> the actual site conditions (see exhibit ss) <br /> On June 16, 2005, Bryant A. Stirrat& Associates submitted the work plan to construct <br /> the new gas system expansion. The system consisted of 11 gas extraction wells in FU03, <br /> 3 gas extraction wells in FU04 and 29 gas extraction wells along the western boundary. <br /> • On June 24, 2005,this LEA conducted routine inspection and noted the following <br /> violations: <br /> 1. PRC 44004—The tonnage of waste received at this facility was as high as <br /> 10,048.9 tons per day. <br /> 2. PRC 44014(b)—The weekly tonnage of waste received at this facility was <br /> as high as 51,486.89 tons. <br /> 3. 27CCR 21600—RDSI has not been amended to include the ADC usage <br /> and waste to cover ratio. <br /> 4. 27CCR 20919.5 —The concentration of methane gas in GP-9 was 48.4%. <br /> The inspection report listed 8 areas of concern for the month of June 2005 (see exhibit tt). <br /> On July 15, 2005, Bryant A. Stirrat& Associates completed the construction of the new <br /> gas system expansion. The system was connected to the existing gas extraction system. <br /> On July 26, 2005, the EHD received a letter from Kevin Basso of Forward Inc. The letter <br /> disputes the violation for tonnages limits listed on the June 24, 2005 inspection report. <br /> The letter states that Forward Inc. is looking for options for the green waste and states <br /> that they would have an RDSI amendment to the EHD by July 31, 2005 (see exhibit uu). <br /> On July 27, 2005,this LEA conducted routine inspection and noted the following <br /> violations: <br /> • 1.
