DRAFT <br /> • On June 17, 2003, the new permit combining the Forward Landfill and Austin Road <br /> Landfill under on Solid Waste Facility Permit. (see exhibit A) <br /> On June 27, 2003, Michael Kith and Ted Tasiopoulis employees of the EHD conducted <br /> routine inspection and noted that the concentration of methane gas in the gas monitoring <br /> probe GP-9 (which replaces GP-2), located near the west boundary, was 18 %. This was <br /> documented under the violation of Gas Control, Section 20919 of Title 27 of California <br /> Code of Regulation(27CCR) (see exhibit B). <br /> On July 29, 2003, Michael Kith and Ted Tasiopoulis employees of the EHD conducted <br /> routine inspection and noted that the concentration of methane gas in the gas monitoring <br /> probes GP-8 (which replaces GP-3) and GP-9 (which replaces GP-2), both of which were <br /> located near the west boundary, was 8% and 18 %, respectively. This was documented <br /> under the violation of Gas Control, Section 20919 of Title 27 of California Code of <br /> Regulation (27CCR) (see exhibit Q. <br /> On July 30, 2003, The Regensis Group, Contractor, of 2210 South Asuza Avenue, West <br /> Covina, California, submitted a plan to install gas extraction system that would be <br /> connected to the existing flare station. <br /> On August 28, 2003, Michael Kith and Ted Tasiopoulis employees of the EHD <br /> • conducted routine inspection and noted that the concentration of methane gas in the gas <br /> monitoring probes GP-9 (which replaces GP-2), located near the west boundary, was 37 <br /> %. This was documented under the violation of Gas Control, Section 20919 of Title 27 <br /> of California Code of Regulation(27CCR) (see exhibit D). <br /> On September 30, 2003, Michael Kith and Ted Tasiopoulis employees of the EHD <br /> conducted routine inspection and noted that the concentration of methane gas in the gas <br /> monitoring probe GP-9 (which replaces GP-2), located near the west boundary, was 37%. <br /> This was documented under the violation of Explosive Gases Control, Section 20919.5 of <br /> 27CCR(see exhibit E). <br /> On October 3, 2003,this LEA prepared and sent to the operator the Notice and Order 03- <br /> 01 for the violation of Explosive Gases Control, Section 20919.5 of 27CCR(see exhibit <br /> F). <br /> On October 24, 2003,this LEA received the progress report for Notice and Order 03-01. <br /> According to the progress report, the landfill gas system expansion project was <br /> completed on October 2, 2003, and the vacuum had been applied to the new extraction <br /> wells (see exhibit G). <br /> On October 24, 2003,Michael Kith and Ted Tasiopoulis employees of the EHD <br /> conducted routine inspection and noted that the concentration of methane gas in the gas <br /> monitoring probe GP-9 (which replaces GP-2), located near the west boundary, was 40%. <br /> • This was documented under the violation of Explosive Gases Control, Section 20919.5 of <br />
