The facility was also cited for lack of daily cover Section 20680 of 27CCR and <br /> for accepting un-treated medical waste Section 20880 27CCR (see exhibit H). <br /> On November 20, 2003, Michael Kith and Ted Tasiopoulis employees of the EHD <br /> conducted routine inspection and noted that the concentration of methane gas in the gas <br /> monitoring probes GP-8 (which replaces GP-3) and GP-9 (which replaces GP-2), both of <br /> which were located near the west boundary, was 9% and 41%, respectively. This was <br /> documented under the violation of Explosive Gases Control, Section 20919.5 of 27CCR. <br /> (see exhibit I). <br /> On December 23, 2003, Michael Kith and Ted Tasiopoulis employees of the EHD <br /> conducted routine inspection and noted that the concentration of methane gas in the gas <br /> monitoring probes GP-8 (which replaces GP-3) and GP-9 (which replaces GP-2), both of <br /> which were located near the west boundary, was 9.5% and 44%, respectively. This was <br /> documented under the violation of Explosive Gases Control, Section 20919.5 of 27CCR <br /> (see exhibit J). <br /> On January 14, 2004, at the request of CIWMB, LEA amended the Notice and Order 03- <br /> 01 so that the violations cited for the Explosive Gas Control were consistent for 3 <br /> consecutive months. <br /> On January 30, 2004, Michael Kith and Ted Tasiopoulis employees of the EHD <br /> • conducted routine inspection and noted that the concentration of methane gas in the gas <br /> monitoring probes GP-8 (which replaces GP-3) and GP-9 (which replaces GP-2), both of <br /> which were located near the west boundary,was 48% and 45%, respectively. This was <br /> documented under the violation of Explosive Gases Control, Section 20919.5 of 27CCR. <br /> The facility was cited for an Area of Concern for Self Monitoring, Section, 44014 (b), <br /> PRC (see exhibit K) <br /> On February 2, 2004, the EHD issued the Amended Notice and Order 03-01 to the <br /> owner/operator of the facility for the violation of the explosive gas control, <br /> 27CCR20919.5 (see exhibit L). <br /> On February 27, 2004, Michael Kith and Ted Tasiopoulis employees of the EHD <br /> conducted routine inspection and noted that the concentration of methane gas in the gas <br /> monitoring probes GP-8 (which replaces GP-3) and GP-9 (which replaces GP-2), both of <br /> which were located near the west boundary, was 45% and 45%, respectively. This was <br /> documented under the violation of Explosive Gases Control, Section 20919.5 of 27CCR <br /> (see exhibit M). <br /> On March 2, 2004, the EHD received a progress report for Amended Notice and Order <br /> 03-01. According to the progress report,the operator/owner proposed to install 52 gas <br /> extraction wells in 3 different phases. All new gas extraction wells would be connected <br /> to the existing flare station. <br /> • <br />
