Violation T27CCR 21600—Report of Disposal Site Information-JTD has not been amended to include the information listed <br /> in T27CCR21600(b)(6)(B),the ADC usage and waste to cover ratio. <br /> a. Submit application and RDSI amendment to include all information listed in T27CCR216000(b)(6)(B)to LEA for <br /> approval. <br /> b. Also see inspection dated 4/20/05,5/25/05,6/24/05,7/27/05, 8/26/05,9/29/05, 10/28/05, 11/28/05, 12/28/05 and <br /> 1/24/06 for previous violation of this section. <br /> c. The proposed JTD amendment received on August 2nd,2005 was rejected by this LEA in a response letter dated <br /> August 23`d,2005 due to conflicting with existing approvals for the facility. <br /> d. The proposed JTD Amendment received on January 1,2006 was rejected by'the LEA in a response letter dated <br /> March 17,2006 due to noncompliance with Title 27 CCR Section 21665. A meeting was held with Forward Inc. <br /> on March 23,2006 regarding this issue. <br /> e. To date,the EHD has not received any additional information regarding the JTD submittal. <br /> 2. AC T27 CCR 20590—Personnel Health and Safety—Observed drivers of refuse vehicles not wearing safety vests for <br /> hard had when they are out of their vehicle at the disposal area. <br /> a. Drivers of refuse vehicles shall wear hard hats and safety vests. <br /> 3. AC T27CCR20650 Ponding observed at north side of deck at closed site. <br /> a. Regrade the surface to promote lateral runoff of precipitaion and to prevent ponding. <br /> 4. AC T27CCR20820-Erosion channels along the drains at the bench have been graded. Operator indicated that more <br /> work shall be done to repair the erosion channels along the bottom of the south slope. <br /> a. Repair all erosions. <br /> b. Provide adequate drainage and erosion control to prevent future erosion, <br /> 5. Violation T27CCR20919.5 The gas extraction system installation is complete and running. The concentrations of methane <br /> gas have improve from last inspection and were as follows: <br /> In GPI 1: 0.2 % in shallow 4.6%in middle 0.1% in deep <br /> In GP12: 3.2%in shallow 33.0%in middle 3.3 % in deep <br /> a.
