The compliance date for Amended Notice and Order 03-01 was November 20,2005. The compliance date had <br /> passed. The facility is still in violation of T27CCR20919.5 Explosive Gas Control. <br /> 8. Notes: <br /> A. The extension of the Amended Notice and Order 03-01 expired on November 20,2005. The compliance date has passed. <br /> This facility is still in violation of T27CCR20919.5 Explosive Gas Control. <br /> On November 23,2005,the EHD issued the Notice and Order 05-01 with the compliance schedule to Forward Inc.for th <br /> l* <br /> violations of PRC 44004 Significant Change,PRC 44104 Terms and Conditions of Permit,and T27 CCR 21600 Report <br /> of Disposal Site Information. The compliance date was December 23,2006. <br /> Notice and Order 03-01 and 05-01 are expired. Due to non-compliance status of the facility,the issues were referred to <br /> the San Joaquin County District Attorneys Office for further enforcement and resolution. <br /> B. Tarps was not being used as ADC at time of inspection. TASW was being used as ADC. <br /> C. Daily cover was adequate at time of inspection except for some daylighting observed. <br /> D. Grading of low areas on top deck of south module is needed. Provide. <br /> E. Load checking program needs to be updated to include the requirement of the removal of the universal waste.
