Violation T27CCR 21600-Report of Disposal Site Information-The JTD has not been amended to include the information <br /> listed in T27CCR21600(b)(6)(B),the ADC usage and waste to cover ratio. <br /> a. Submit application and RDSI amendment to include all information listed in T27CCR216000(b)(6)(13)to LEA for <br /> approval. <br /> b. Also see inspection dated 4/20/05, 5/25/05,6/24/05, 7/27/05, 8/26/05,9/29/05, 10/28/05, 11/28/05, 12/28/05, <br /> 1/24/06,2/08/06,3/30/06,4/21/06,5/18/06, and 6/23/06 for previous violation of this section. <br /> c. The proposed JTD amendment received on August 2"d,2005 was rejected by this LEA in a response letter dated <br /> August 23rd,2005 due to conflicting with existing approvals for the facility. <br /> d. The proposed JTD Amendment received on January 1,2006 was rejected by the LEA in a response letter dated <br /> March 17,2006 due to noncompliance with Title 27 CCR Section 21665. A meeting was held with Forward Inc. <br /> March 23,2006 regarding this issue. <br /> e. The proposed JTD Amendment received on May 23,2006 was rejected by this LEA in a response letter dated June <br /> 22,2006 because it did not meet Title 27 CCR Section 21665. <br /> f, To date,the EHD has not received any additional information regarding the JTD submittal. <br /> 2.AC T 27 CCR 20690-Alternate Daily Cover-Visqueen was being used as ADC instead of tarps. Exposed waste <br /> observed through torn visqueen. Operator indicated that vis-queen was being used on an interim basis due to tarps <br /> being damaged by a fire and new tarps are to arrive today or this following Monday. <br /> a. Visqueen is not considered an adequate material to be used as ADC. Discontinue use. <br /> b. Provide adequate ADC. <br /> 3.AC T 27 CCR 20690-Alternate Daily Cover-At class Il area,TASW ADC was being covered by additional cover <br /> consisting of contaminated soil(including large chunks of concrete)where it appeared that no waste was being <br /> disposed(cover being placed on top of cover). During inspection,operator indicated that soil was placed on top of <br /> TASW at areas where waste was not going to be placed for some time and it appeared as if cover was being placed <br /> on top of cover. According to the operator,the class III active face was covered with tarp/visqueen and clean soil <br /> from the pit area. During exit interview,operator indicated that this additional cover was being placed on top of <br /> TASW to provide better stability for equipment. <br /> a.- Discontinue adding extraneous soil cover to TASW ADC. ADC shall be used no more than necessary to meet the <br /> minimum requirements of T 27 CCR 20686 and 20690(b). <br /> b. Contaminated soil including large chunks of concrete is not considered suitable for use as ADC. Discontinue use. <br /> A <br /> INSPECTOR SIGNATURE: DATE: C) <br />
