AC T27 CCR 20919.5-Explosive Gas Control -The gas extraction system installation is complete and running. The <br /> concentrations of methane gas have improve from last inspection and measured less than 5 %methane by volume of <br /> air as follows: <br /> In GPI 1: 0.1 %in shallow 0.1 %in middle 0.1 % in deep <br /> In GP12: 0.1 % in shallow 0.1 % in middle 3.3 % in deep <br /> a. Ensure that the concentration of explosive gas at the property boundary is below 5 %by volume. <br /> WF <br /> 5. Notes: <br /> A. On November 23,2005,the EHD issued the Notice and Order 05-01 with the compliance schedule to Forward Inc. for <br /> the violations of PRC 44004 Significant Change,PRC 44104 Terms and Conditions of Permit, and T27 CCR 21600 <br /> Report of Disposal Site Information. The compliance date was December 23,2005. <br /> Notice and Order 05-01 is expired. Due to non-compliance status of the facility,the issues were referred to the San <br /> Joaquin County District Attorneys Office for further enforcement and resolution. <br /> B. Records indicated 620 vehicle trips and 8,657 tons was received occurred on 7/26/06. The facility closed early at 5:50 <br /> PM to avoid violating the SWFP Permitted Traffic Volume of 620 Trips per Day and the Permitted Maximum Tonnage <br /> of 8,668 Tons per Day. <br /> C. Special occurrence records indicated that on 7/23/06,a fire occurred at working face from 2 pm to 10:30 pin. The fire <br /> was extinguished at 10:30 pm. Operator indicated that the tarps used at ADC was damaged due to the fire. Vis-queen <br /> was being used temporarily until new tarps are to arrive either today or this following Monday. <br /> D. Load checking program needs to be updated to include the requirement of the removal of the universal waste. Operator <br /> furnished training records for personnel for procedures of handling U-wastes. <br /> E. Observed drivers of refuse vehicles wearing safety vests for hard had when they are out of their vehicle at the disposal <br /> area. <br /> AML F.
