11 C t Polyethylene• Fittings TA Receiving Inspection Upon arrival by truck shipment, the Ausenco Vector CQA monitor performed the receiving inspection of the HDPE piping, which consisted of: • observing that the pipe is the proper size and dimensions with the specified perforations; • visually inspecting the pipe for any damage that may have occurred during transportation and handling; • observing and documenting that storage of the pipe was in accordance with the project specifications; and • documentation required by the specifications was received. Documentation for the HDPE pipe is included in Appendix M. 6.2 Installation Ausenco Vector's CQA monitor observed that the pipe was installed at the locations specified in the construction drawings and no damage occurred during the placement and backfill around and over the pipe. Ausenco Vector's CQA Monitor spot -monitored the fusion welding operations to ensure that the welding followed the project specifications. In addition to the pipe, Ausenco Vectors CQA Monitor observed that the installation of the HDPE tanks were as specified in the specifications and construction drawings. Observation and testing of the soils around the sump, including the termination berm, was done as part of the subgrade preparation and engineered fill construction. 7 Conclusion Construction of the liner and LCRS components of WMU FU -10 was completed in September 2010. Based on our observations and testing during the construction of the above referenced portion of the WMU FU -10 composite liner system at the Forward Landfill, it is Ausenco Vector's professional judgment that the construction was completed in general accordance with the project plans and specifications, CQA Manual, applicable regulations, and approved design modifications.
