11 <br />C <br />t <br />Polyethylene• Fittings <br />TA Receiving Inspection <br />Upon arrival by truck shipment, the Ausenco Vector CQA monitor performed the receiving inspection <br />of the HDPE piping, which consisted of: <br />• observing that the pipe is the proper size and dimensions with the specified perforations; <br />• visually inspecting the pipe for any damage that may have occurred during transportation and <br />handling; <br />• observing and documenting that storage of the pipe was in accordance with the project <br />specifications; and <br />• documentation required by the specifications was received. <br />Documentation for the HDPE pipe is included in Appendix M. <br />6.2 Installation <br />Ausenco Vector's CQA monitor observed that the pipe was installed at the locations specified in the <br />construction drawings and no damage occurred during the placement and backfill around and over <br />the pipe. Ausenco Vector's CQA Monitor spot -monitored the fusion welding operations to ensure that <br />the welding followed the project specifications. <br />In addition to the pipe, Ausenco Vectors CQA Monitor observed that the installation of the HDPE <br />tanks were as specified in the specifications and construction drawings. Observation and testing of <br />the soils around the sump, including the termination berm, was done as part of the subgrade <br />preparation and engineered fill construction. <br />7 Conclusion <br />Construction of the liner and LCRS components of WMU FU -10 was completed in September 2010. <br />Based on our observations and testing during the construction of the above referenced portion of the <br />WMU FU -10 composite liner system at the Forward Landfill, it is Ausenco Vector's professional <br />judgment that the construction was completed in general accordance with the project plans and <br />specifications, CQA Manual, applicable regulations, and approved design modifications. <br />Date: October 2010 <br />USVC0004800 <br />Final Construction Quality Assurance Report <br />25 <br />
