• If the person collecting -the sample does not deliver the samples containers for <br /> shipment, the first block for Relinquished By_______, Received By________., must <br /> be completed. <br /> The samples will be shipped to the laboratory by commercial carrier. The original <br /> COC form is placed in the shipping container and the shipping container is sealed <br /> prior to being given to the carrier. The carbonless copy of the COC form is sent to the <br /> field file. The carrier waybill will serve as an extension of the COC record between <br /> the final field custodian and receipt in the laboratory. <br /> Upon receipt in the laboratory, the quality control (QC) coordinator or a designated <br /> representative opens the shipping containers, compares the contents with the COC <br /> record, and signs and dates the record. Any discrepancies are noted on the COC form. <br /> • If discrepancies occur, the samples in question are segregated from normal sample <br /> storage and the field personnel immediately notified. <br /> • COC records are maintained with the project records by becoming part of the <br /> data/report package. <br /> In addition to initiating the COC form, field personnel are responsible for uniquely identifying <br /> (required on the COC form) and labeling samples, providing proper preservation, and packaging <br /> samples to preclude breakage during shipment. <br /> Every sample label includes the following: <br /> • A unique sample number <br /> • Sample Location <br /> • Sampling date and time <br /> • Name of person collecting the sample <br /> • Method of sample preservation/conditioning <br /> Shipping containers are sealed at the ends with tape prior to shipment. <br /> The final step in providing information to the laboratory is the request for analysis. The request <br /> for analysis section is completed by the field personnel on the COC record. The request for <br /> analysis form is provided so that analytical requirements are defined and sample holding times <br /> are not exceeded. A copy of the COC and request for analysis form is maintained in the field <br /> data sheet/log. <br /> 7.2.2 Laboratory Sample Handling <br /> The first step in the laboratory receipt of samples is to complete the log-in form. The QC <br /> coordinator or a designated alternate accepts the shipment and notifies the laboratory manager or <br /> designated representative of the incoming samples. <br />
