Final Construction Quality Assurance Report-WMU FU -13 <br />Forward Landfill <br />• Location and Frequency — GLA's CQA Monitor selected the locations of approximately one <br />sample per 500 linear feet of seam. Within WMU FU -13, a total of 49 destructive samples were <br />collected and tested from approximately 23,200 linear feet of seam, resulting in a frequency of <br />1 test per 473 linear feet. Additional destructive samples were obtained if there were failures <br />or if there was an indication of overheating, contamination, or other adverse conditions. <br />Sampling Procedure - Comanco cut the samples as directed by GLA's CQA Monitor. GLA's CQA <br />Monitor observed the cutting and assigned sample numbers, and reviewed sample information <br />(sample number, seam number, seaming apparatus, operator identification, location, date, and <br />time welded). GLA's CCW Monitor recorded the samples collected and sample location on the <br />panel layout drawing. <br />The holes in the geomembrane resulting from destructive seam sampling were covered by <br />Comanco immediately after sampling. The repair was completed by placing an extrusion <br />welded patch of the same material. The continuity of the new seams in the repaired area was <br />tested with the same procedure as described in the vacuum box testing procedures discussed <br />previously. <br />• Size of Samples - Prior to testing in GLA's field laboratory, two samples, one at each end of the <br />actual sample location, were cut for pre -testing. If both specimens passed the pre-test, the <br />sample was taken to the field laboratory for testing. The size of the destructive samples was <br />approximately 12 inches wide by 42 inches long. <br />Laboratory Testing - The destructive samples were tested with a certified, calibrated <br />tensiometer according to the requirements of ASTM D-6392. After seam strength tests for the <br />destructive samples were obtained, the results were given verbally to the Contractor and were <br />also reviewed by GLA's CCW Officer. The results of the destructive seam tests performed by <br />GLA are presented within Appendix I. <br />5.3.3 Defects and Repair <br />5.3.3.1 Identification <br />GLA's CCW Monitor checked seams and non -seam areas of the geomembrane for any defects, <br />holes, blisters, undispersed raw materials, and for signs of contamination by foreign matter. <br />Suspect areas were distinctively marked for repair, with a description of the required type of <br />repair and the repair location recorded in the Repair Logs. No additional materials were placed <br />over the geomembrane until GLA's CQA Monitor approved the repair. <br />25 Geo -Logic <br />ASSOCIATES <br />
