File No. 20-1008-36 <br /> July 14, 1995 <br /> 8 DOCUMENTATION <br /> All sampling, testing, and reduction of test data shall be completely documented. Information <br /> shall be recorded on standard forms. Documentation shall include a daily log of project activities <br /> and appropriate logs, forms, and photographs. Sample forms are presented in Appendix C. <br /> Each log shall be signed by the person who prepared it. Items to be included in the daily log are: <br /> • Project identification (Name) Visitors on site <br /> (Kleinfelder, Client, Agency, etc.) <br /> • Project number Contractor progress or problems <br /> • Field activity of technician The outline of any communications <br /> • General work activity of contractor Weather Conditions <br /> • Unusual events Time on site <br /> • Changes to plans or specifications Samples taken to lab <br /> Copies of the daily logs should be sent to the Chief QC Technician daily. Copies of all <br /> construction records shall be collected weekly and maintained on-site by the Project Manager for <br /> the County. <br /> As part of field activities, a photographic record will be maintained. Photographs will be in color. <br /> Photographs will be taken of the general site layout, field equipment, completed construction <br /> elements, etc. If test pits or trenches are dug, photographs will be taken of the subsurface <br /> profiles. <br /> Each photograph will have a description which includes job number and phase, date, technician ID <br /> number or letter, and the daily photograph number. An example of this designation is 20-1008- <br /> 36/May 10, 92/1943/7.
