2.4.6 Project Records <br /> Various other project record forms are coinpleted as needed. The use of project record forms is <br /> limited to the purpose(s) for which they are intended. Persons preparing project record forms <br /> shall complete them by filling in the blanks provided, and shall sign and date each project record <br /> form. The originals of project record forms shall be submitted to the CQA Monitor, who shall <br /> copy and distribute them as appropriate and file and maintain them as Project Documents. <br /> 2.5 Documentation and Control of Nonconformances <br /> Any item, activity, work product, or Project Document that does not conform to the requirements <br /> of the Contract Documents or applicable laws and regulations is handled as a nonconformance. <br /> Major goals of the CQA program are to minimize the number and severity of nonconformances, <br /> to resolve nonconformances as quickly, efficiently, and economically as is practical, and to <br /> prevent nonconformances from adversely affecting the quality or performance of the completed <br /> project. <br /> 2.5.9 Observation of Nonconformances <br /> All parties involved in the Extension of the Closure Cap of the CHLF have a responsibility to <br /> identify and report nonconformances. If a suspected nonconformance is discovered or observed <br /> during construction or in the product, job-related materials, documentation, or elsewhere, the <br /> person discovering or observing the suspected nonconformance will promptly report it to the <br /> CQA Monitor. The CQA Monitor will investigate the report and, if it is credible, notify the CQA <br /> Engineer and the Resident Engineer as soon as it is practical to do so. The Resident Engineer will <br /> then notify contractor. The Resident Engineer will designate CQA Monitor the authority to stop <br /> contractor's work if non conformance work area <br /> 2.5.2 Determining Extent of Nonconformances <br /> ' Whenever a nonconformance is discovered or observed in the construction process, product,job- <br /> related materials, documentation, or elsewhere, the CQA Monitor will determine the extent of <br /> the nonconformance. The extent of the deficiency may be delineated by additional sampling, <br /> testing, observations,review of records, or other means, as appropriate. <br /> 2.5.3 Documenting Nonconformances <br /> Nonconformances shall be documented in writing on the Daily Report, other logs or reports, and <br /> elsewhere, as appropriate. Documentation must occur immediately upon determining the extent <br /> ' of the nonconformance. For those nonconformances that are serious or complex in nature, or that <br /> require engineering evaluation, the CQA Monitor shall issue a Nonconformance Report to the <br /> CQA Engineer, Resident Engineer, Contractor, and County within one working day after the <br /> extent of the nonconformance has been investigated. <br /> CH CQA Plan 041712 Rev2a.doc 2-6 Construction Quality Assurance Plan <br />
