Z5.4 Corrective Measures <br /> Simple or routine nonconformances are those for which a nonconformance report is not issued. <br /> Corrective measures for simple or routine nonconformances normally will be determined from <br /> the requirements of the Contract Documents. If no relevant or adequate corrective measures can <br /> be so determined, the CQA Monitor and the CQA Engineer will notify the Resident Engineer to <br /> inform the contractor to select one or more standard-of-practice construction methods to resolve <br /> the nonconformance. In the event that the CQA Monitor, CQA Engineer, and Contractor are <br /> unable to agree upon such methods, the CQA Monitor shall issue a Nonconformance Report and <br /> the matter will be upgraded to a nonconformance that is serious or complex, or that requires <br /> engineering evaluation. <br /> For nonconformances that are serious or complex, or that require engineering evaluation, the <br /> Resident Engineer will respond to the Nonconformance Report by corrective action. The <br /> Resident Engineer shall return the Nonconformance Report with the corrective-action design <br /> attached to the CQA Monitor, CQA Engineer, Contractor, and County. If the County finds that <br /> no concurrence by regulatory agencies is needed, the Contractor shall carry out the corrective <br /> action. If regulatory concurrence is needed, implementation of the corrective action will be <br /> deferred until concurrence has been obtained. <br /> 2.5.5 Verification of Corrective Measures <br /> When corrective measures are complete, the Contractor shall so advise the CQA Monitor, who <br /> will verify that those measures substantially conform to the corrective action design and that they <br /> have been properly completed. The CQA Monitor shall provide evidence to substantiate <br /> verification by observations, retesting, photographs, and other appropriate means. The CQA <br /> ' Monitor shall document the corrective measures in writing on the Daily Reports, other logs and <br /> forms, and the original Nonconformance Report. The verification shall be reviewed by the CQA <br /> Engineer. The updated Nonconformance Report shall be signed and dated by the CQA Monitor <br /> and the CQA Engineer upon review, and shall be filed and maintained as a Project Document. <br /> 2.6 Construction Surveys <br /> Survey completed work for the record drawings at the following locations: <br /> 1. Top of subgrade; <br /> 2. Top of vegetative soil layer; <br /> 3. Line, inverts and outlets and elevations, culverts, ditches and concrete structures; and <br /> 4.
