The operator will immediately commence and thereafter maintain <br /> a log of special occurrences . <br /> D. Provisions : <br /> 1. The following provisions must be met : <br /> The operator must provide safety helmets and respirators for <br /> all employees working the equipment at the site by December <br /> 1, 1977 . <br /> OKI Adequate steps must be taken to provide a barrier across the <br /> eastern front of the site, (Manthey Road side) . The barrier <br /> must be designed to discourage unauthorized entry by persons <br /> or vehicles and be completed by December 1, 1977 . <br /> Additional cover is needed in the area east of the present <br /> working surface. Provide cover material that has been com- <br /> pacted to a minimum thickness of six (6) inches by December <br /> 1, 1977 . <br /> 2, This permit is subject to review by the enforcement agency, and <br /> may be suspended, revoked, or modified at any time for sufficient <br /> cause. <br /> E. Self-Monitoring Program: <br /> 1. The following items shall be monitored by the operator of this <br /> facility or his agent and records shall be kept and made avail- <br /> able to the enforcement agency upon request : <br /> a. Quantity and types of wastes received at the site per day <br /> and per week. <br /> b . Area of site utilized. <br /> c. Quantity and types of wastes salvaged per month. <br /> d. Number of vehicles utilizing the site per day and per week. <br />
