V T22 CCR 66265.77- Failed to report releases to the Department- On going release of oil from refuse vehicle crusher gate <br />was observed in parking lot west of green waste operations where vehicle dismantling operations had been taking place. An <br />oil release was observed in dirt underneath retired equipment at northwest corner of facility next to easement. <br />Report all hazardous waste releases to the Department immediately. <br />2.) V T22 CCR 662653.173(a)- Failed to keep containers of HW closed except when adding/removing HW- An open plastic <br />5 -gallon bucket was observed holding petroleum fuel in parking lot west of green waste operations where vehicle <br />dismantling operations had been taking place. <br />Keep containers of hazardous waste closed except when adding/removing hazardous waste. Transfer all hazardous waste to <br />closed container immediately. <br />3.) AC T14 CCR 17409.5- Load Checking- According to load checking records, customer tracking and load checking is not being <br />performed on Saturdays and Sundays. <br />All incoming waste shall be inspected for hazardous waste. Load checks are to be documented daily under Permit item 17.) - <br />LEA Conditions (G). <br />4.)
