FACILITY <br /> PERMIT Facility/Permit Number: <br /> SOLID WASTE 39-AA-0016 <br /> Self Monitoring: <br /> The owner/operator shall submit the results of this self-monitoring program within 30 days of the end of the reporting period <br /> unless otherwise specified: <br /> Program Reporting Frequency Agency Reported To <br /> a. Types and quantities of waste entering the site per day and per Quarterly Enforcement Agency (EA) <br /> month. <br /> b. Types and quantities of goods recycled/salvaged per month. Quarterly EA <br /> c. Number of vehicles entering the site per day and per month. Quarterly EA <br /> d. Copies of all written complaints regarding this facility and a Quarterly EA <br /> description of the actions taken to resolve these complaints. <br /> e. Results of the hazardous waste screening program. Quarterly EA <br /> 17. EA Conditions: <br /> a. This facility shall comply with all federal, state and local requirements and enactments including all mitigation measures <br /> given in any certified environmental document filed pursuant to Public Resources Code, Section 21081.6. <br /> The owner/operator shall maintain copies of this and all other permits issued for the operation of this facility, a copy of the <br /> most recent inspection report, a copy of the most recent printing of the State Minimum Standards for Solid Waste <br /> Handling and Disposal [California Code of Regulations (CCR), Title 14, Division 7, Chapter 3] . <br /> c. This permit is subject to review by the EA and may be suspended, revoked or modified at any time for sufficient cause. <br /> d. The EA reserves the right to suspend or modify waste receiving operations when deemed necessary due to an <br /> emergency, a potential health hazard or the creation of a public nuisance. <br /> e. The operator shall maintain a log of special occurrences. The log shall include, but is not limited to: Fires, explosions, <br /> earthquakes, discharge of hazardous substances, significant injuries, accidents or property damage. Each log entry <br /> shall be accompanied by a summary of any actions taken by the operator to mitigate the occurrence. The operator shall <br /> maintain this log at the facility so as to be available at all times to site personnel and EA personnel. <br /> f. Any change that would cause the design or operation of the facility not to conform to the terms and conditions of this <br /> permit in prohibited. Such a change may be considered a significant change, requiring a permit revision. In no case <br /> shall the operator implement any change without first submitting a written notice of the proposed change to the EA at <br /> least 120 days in advance of the change. <br /> g. This facility has a permitted capacity of 500 tons per day and shall not receive more than this amount without a revision <br /> of this permit. <br /> h. Safety equipment shall be utilized as described in the Report of Station Information. <br /> A copy of this permit shall be maintained at the facility. <br /> tIm <br />
