Any change that would cause the design or operation of <br /> the facility not to conform to the conditions of this <br /> permit will be considered a significant change and will <br /> require a permit revision. <br /> 3 . The facility has a permitted capacity of 700 tons per day <br /> and shall not receive more than this amount without first <br /> obtaining a revised permit. <br /> 4 . A change in the operator would require a new permit. <br /> S. This modification of the solid waste facilities permit <br /> supersedes the permit originally issued on September 13 , <br /> 1983. The permit has been modified as a result of 5 year <br /> permit review, and is intended to update the previous <br /> permit. <br /> PROVISIONS <br /> 1. This permit is subject to review by the Local Enforcement <br /> Agency, and may be modified, suspended, or revoked for <br /> sufficient cause. <br /> 2 . Safety equipment shall be provided for and used by all <br /> employees at this facility in accordance with the <br /> Lovelace Transfer Station Employee Work Rules. <br /> SELF-MONITORING PROGEM <br /> The following items shall be monitored by the operator of facility <br /> or his agent. Records, including but not limited to the following <br /> items, shall be kept and made available to the Local Enforcement <br /> Agency at the intervals specified: <br /> 1. Submit tonnage records of wastes received each day <br /> (daily) on a quarterly basis. <br /> 2. Quantities and types of materials recycled and/or <br /> salvaged shall be reported to the Local Enforcement <br /> Agency on a quarterly basis. <br /> 3 . A log of special occurrences, i.e. , fires, explosions, <br /> accidents, hazardous wastes, etc. , shall be maintained <br /> and reported to the Local Enforcement Agency on an annual <br /> basis. <br /> 4 . Results of the hazardous waste screening program shall be <br /> reported to the Local Enforcement Agency on an annual <br /> basis. <br /> 4 <br />
