2 . Any change that would cause the design or operation of this <br /> facility riot to conform to the conditions of this permit may <br /> be considered a significant change and may require a permit <br /> revision. <br /> 3 . The facility has a permitted daily capacity of 300 tons per <br /> operalina day and shall not receive more than this amount <br /> without first obtaining a new permit . <br /> 4. A change in the operator would require a new permit . <br /> 5 . All existing underground storage tanks will meet the <br /> requirements of California Code of Regulation Title 23 , <br /> Chapter 3, Subehapter 16 . <br /> 6 . The station's underground tank soil bio-remediation <br /> treatment area must be approved by both the Regional Water <br /> Quality Control Board and the State Department of Health <br /> Services . This area cannot be used to service any off-site <br /> underground tank waste material. <br /> IMMUIM: <br /> 1 . This permit is subject to review by the Local Enforcement <br /> Agency, and may be modified, suspended, or revoked, for <br /> sufficient cause after a hearing . <br /> 2 . Safety equipment shall be provided for all employees <br /> operating equipment at this facility including but not <br /> limited to the following: <br /> a. respirator's <br /> b. safety helmets <br /> C. safety shoes or boots <br /> d. gloves <br /> e . ear protection <br /> f. goggles or mask <br /> g. protective clothing if needed <br /> OELF-MQNITQRING PROGRAM: <br /> The following items shall be monitored by the operator of this <br /> facility or his agent . Records , including but not limited to the <br /> following items, shall be kept and made available to the LEA at <br /> intervals specified. <br /> 1 . Quantities and types of waste received shall be reported to <br /> the LEA on a quarterly basis . <br /> 2 . Quantities and types of goods recycled and/or salvaged shall <br /> be reported to the LEA on a quarterly basis . <br />
