A record of such training history-must be maintained on site as well as an employee attendance sheet with <br /> printed and signed names. <br /> x 17408.6- Maintenance Program <br /> Comments: There is no designated water source for emeregency use(i.e. eye wash station). <br /> 1. All aspects of the operation shall be maintained in a state of good repair which includes having an <br /> an adequate water supply for emergency use. <br /> 2. Records of incoming and outgoing weights and volumes shall be maintained on site. <br /> 2. Have such records available during inspections. <br /> x 17414(d)-Special Occurrence <br /> Comments: There was no daily log book or log of special occurrences. <br /> 1. The operator shall maintain a daily log book or log of special occurrences encountered during operations <br /> and methods used to resolve problems arising from these events, including details of all incidents that <br /> required implementing emergency procedures. <br /> 2. Special occurrences include:fires, injury, property damage, accidents, explosions, receipt or rejection of <br /> prohibited wasts, etc. <br /> 3. Create and maintain a log book or log of special occurrences. <br /> x 17414(e)- Records of Written Complaints <br /> Comments: There was no record of written complaints on site. <br /> 1. The operator shall record any written public complaints received. <br /> 2. The record shall include: (1)the nature of the complaint, (2)the date the complaint was received, (3)the <br /> name, address, and telephone number of the complaintant(if available), and (4)any actions taken to <br /> respond to the complaint. <br /> 3.
