Routine quarterly transfer station inspection conducted on this date and the following was noted: <br /> 1. Prevous area of concern regarding load checking has been corrected. A newly formatted load <br /> checking form was provided. <br /> 2. Previous area of concern regarding personnel health and safety has been corrected. An IIPP <br /> was provided. <br /> 3. Previous area of concern regarding training has been corrected. Training documents <br /> (attendance, agenda, information sheet, and quiz)were provided. <br /> 4. Previous areas of concerns regarding special occurrence and records of written complaints <br /> (same form)has been corrected. Facility was instructed to add the following to the form: the <br /> name, address, and telephone number of the complaintant. <br /> 5. Portable bathrooms were observed during the inspection. <br /> 6. Facility contact, Kevin Clutter, will provide the required documentation (tonnange, loadchecking, <br /> training, special occurrences, etc.)for each quaterly inspection. <br />
