The design and operation of this facility must comply with all applicable State Minimum Standards for solid waste handling as specified in Title 14,California Code of Regulations. 2. The operator shall maintain a log of special/unusual occurrences. This log shall include,but is not limited to,fires,explosions, the discharge and disposition of hazardous or un-permitted wastes,and significant injuries,accidents or property damage. Each log0 entry shall be accompanied by a summary of any actions taken by the operator to mitigate the occurrence. The log shall be available to site personnel and the EA at all times. 3. The operator shall comply with all waste discharge requirements,Clean-up and Abatement Orders,monitoring,remediation schedules and related requirements of the local California Regional Water Quality Control Board(CRWQCB). Degradation of water connected to this site shall be remediated in the manner specified y the CRWQCB. 4. Additional information concerning the design and operation of this facility must be furnished upon the request of the LEA. 5. Forward Resource Recovery Facility must comply with the conditions of approval of Use Permit UP-00-07. , 6. The LEA reserves the right to suspend or modify waste receiving and handling operations when deemed necessary due to an emergency,a potential health hazard,or the creation of a public nuisance. 7. Any change that would cause the design or operation of the facility not to conform to the terms and conditions of this permit is prohibited. Such a change may be considered a significant change,requiring a permit revision. In no case shall the operator implement any change without first submitting a written notice of the proposed change,in the form of an RFI amendment,to the LEA at least ISO days in advance of the change. 8. A change in operator for this site will require a new solid waste facility permit. The LEA shall be notified in writing of any change in ownership or operator of the facility. 9. The facility shall meet the design and operational standards of the California Code of Regulations,Title 14(CCR 14),Sections 17400-17564,and Title 14(CCR 14),Sections 17850-17870. 10. The transfer station facility shall maintain and keep all stockpiles under 12 feet in height within the transfer building. 11. Physical Contaminants and refuse removed from feedstock,compost,or chipped and ground material shall be removed from the site within 7 days and transported to a disposal facility. 12.. All stockpiling of compostable organic material for a combined period of time greater than six months is considered disposal and must be either removed from the site or processed for composting operations. 13. Wastewater shall not be allowed to discharge off the property or into any waterway except as allowed by NPDES permit. 14. All incoming waste shall be inspected for hazardous waste. If hazardous waste is found it shall be handled as hazardous waste in accordance with all applicable State standards. 15. In accordance with Title 14(CCR 14),Section 17867.5. Operators shall ensure that all personnel assigned to the operation shall be trained in subjects pertinent to operations and maintenance,physical contaminants and hazardous materials recognition and screening,with emphasis on odor impact management and emergency procedures. A record of such training shall be maintained on the site. All personnel working with the load screening program shall be trained to recognize hazardous wastes,and shall be trained for safety and evacuation procedure 16. This permit is subject to review by the LEA and may be modified,suspended,or revoked for sufficient cause following a hearing. 17.
