Within 48 <br /> hours of being provided a signed copy of the report,the owner or operator shall provide a description of each <br /> corrective action taken or to be taken and shall sign and date the report, acknowledging the identified compliance <br /> issues. <br /> CORRECTIVE ACTION: Ensure all designated operator inspection reports are completed and signed within the <br /> required time frame in the future. <br /> 2)OBSERVATION:The designated operator failed to verify that all employees have been trained.According to <br /> training records found on site, employee training was last performed on 10/9/2018. <br /> REGULATION GUIDANCE: During the inspection, the designated operator shall review the employee training <br /> records and verify that all facility employees have been trained. <br /> CORRECTIVE ACTION: Ensure that designated operators performing inspections at this facility are including all of <br /> the required information on the reports. <br /> 3)OBSERVATION:The designated operator inspections for the following months were performed more than 30 <br /> days after the previous inspection: <br /> - Initial inspection on 6/28/2019, next inspection performed on 7/31/2019(33 days) <br /> - Initial inspection on 7/31/2019, next inspection performed on 9/3/2019 (34 days) <br /> - Initial inspection on 11/19/2019, next inspection performed on 12/31/2019 (42 days) <br /> REGULATION GUIDANCE: The designated operator visual inspections shall be conducted within 30 days or less of <br /> the previous inspection. <br /> CORRECTIVE ACTION: Ensure that all designated operator inspections are performed within the required time <br /> frames. <br /> 4)OBSERVATION:The designated operator failed to check and note all required items on the visual inspection <br /> reports.The incorrect and missing information included the following from the inspection reports: <br /> -The"NA"in sections VII through XI were not explained. <br /> -Missing CERS ID <br /> -The designated operator answered the question regarding sump sensor locations but did not list which sumps were <br /> visually inspected. <br /> -The designated operator documented visual inspections of the sumps but left the question regarding sump sensor <br /> FA0000670 PR0232257 SCO01 06/29/2020 <br /> EHD 23-01 Rev.9/20/2019 Page 6 of 8 Underground Storage Tank Program OIR <br /> 1868 E.
