S0 JOA U 1 Environmental Health Department <br /> Underground Storage Tank Program Inspection Report <br /> Facility Name: Facility Address: Date: <br /> QUIK STOP MARKET#3148* 205 W LOCKEFORD ST, LODI June 29, 2020 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> locations blank. <br /> -The designated operator documented visual inspections of the UDCs but left the question regarding UDC sensor <br /> locations blank. <br /> REGULATION GUIDANCE: During the visual inspection,the designated operator shall check that all required <br /> testing and maintenance for the UST system have been completed and shall check all required items on the report. <br /> CORRECTIVE ACTION: Ensure that designated operators performing visual inspections at this facility are including <br /> all of the required information on the reports. <br /> This is a repeat violation, Class II. <br /> 315 HSC 25291(e)Water in secondary containment not removed, analyzed, and properly disposed of(pre-Jul 2003). <br /> OBSERVATION:Approximately 18 gallons of liquid was observed in the 87, 89, and 91 fill sumps. <br /> REGULATION GUIDANCE: If water could enter into the secondary containment by precipitation or infiltration, it <br /> must be removed and disposed of properly. <br /> CORRECTIVE ACTION: The service technician removed 6 gallons of liquid from each of the sumps. Ensure that all <br /> sumps and annular spaces are maintained free of liquid. <br /> This was corrected on site. <br /> This is a repeat violation, Class II. <br /> Overall Inspection Comments: <br /> The monitoring system certification on 6/29/2020. The testing was not witnessed in its entirety. Looked inside all <br /> sumps and UDCs. Verified proper sensor locations. Verified spill bucket 5 gallon capacity. An inspection <br /> checklist was provided to the facility operator on the day of inspection. A complete report was finished on <br /> 6/30/2020 and replaces the initial checklist. <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, within 30 days of receiving the complete inspection report. <br /> To minimize person to person contact EHD is choosing to write the name of person receiving the report instead <br /> of having them sign. Starting September 1, 2018, all in-office CERS help will be provided at EHD hourly rate <br /> ($152).
