Failing to verify that all facility employees have been trained, in violation of <br /> 11 California Code of Regulations,title 23 section 2715(c)(6); <br /> 12 n. Failing to inform the local agency of any change of designated underground <br /> 13 storage tank operator(s)no later than thirty(30)days after the change,in violation of California Code <br /> 14 of Regulations,title 23 section 2715(a); <br /> 15 o. Failing to provide training by a designated underground storage tank operator for <br /> 16 facility employees on the proper operation and maintenance of the underground storage tank system <br /> 17 once every twelve(12)months,in violation of California Code of Regulations,title 23 section <br /> 18 2715(f); <br /> 19 (INCLUDING VIOLATIONS FROM SUPERIOR COURT CASE CV027333) <br /> 20 p. Failing to submit Underground Storage Tank Permit Application-Form A and <br /> 21 Underground Storage Tank Permit Application-Form B,in violation of California Code of <br /> 22 Regulation,Title 22,section 2711(b); <br /> 23 q. Failing to maintain evidence of financial responsibility for taking corrective action <br /> 24 and from compensating third parties for bodily injury and property damage caused by a release from <br /> 25 an underground storage tank system, in violation of Health and Safety Code section 25292.2(a); <br /> 26 r. Failing to test secondary containment systems installed after January 1, 2001, <br /> 27 within six months of installation,in violation of Title 22 of the California Code of Regulations <br /> 28 section 2637(a); <br /> -3- <br /> PERMANENT INJUNCTION AND FINAL JUDGMENT PURSUANT TO STIPULATION <br />
