The following is an itemized list of hazardous materials violations that have not been <br /> addressed for DUQUE BROTHERS INC as of July 06, 2020. <br /> Open violations from May 21, 2020 inspection <br /> Violation#2 -Failed to complete and electronically submit a hazardous materials business plan. <br /> The facility's business plan was not submitted electronically to the California Environmental Reporting System <br /> (CERS). As of January 1, 2013, all businesses were required to submit all business plans electronically for 2018, <br /> 2019, and 2020. Immediately log into the CERS at http://cers.calepa.ca.gov/, enter the correct or updated <br /> information, and submit to the EHD for approval. <br /> ❑ This violation was corrected ❑ This violation will be corrected by (date): <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation: <br /> Violation #3 -Failed to establish and implement a hazardous materials business plan. <br /> A Hazardous Materials Business Plan has not been established or implemented for 2018, 2019, and 2020. A <br /> Hazardous Materials Business Plan shall be established and implemented for emergency response to a release or <br /> threatened release of a hazardous material when storing hazardous materials at or above the thresholds quantities <br /> of 55 gallons for materials that are liquids, 500 pounds for solids, or 200 cubic feet for compressed gas, at standard <br /> temperature and pressure. Immediately log into the California Environmental Reporting System (CERS) at <br /> http://cers.calepa.ca.gov/, enter the required information, and submit for review by the EHD. <br /> ❑ This violation was corrected ❑ This violation will be corrected by (date): <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation: <br /> Violation#5 -Failed to complete and/or submit hazardous materials inventory for all reportable materials on <br /> site. <br /> Three 275 gallon totes of diesel exhaust fluid, one 278 cubic foot cylinder of compressed oxygen, two 120 gallon <br /> tanks of used oil, two 200 gallon tanks of new oil, two 55 gallon drums of antifreeze, and one 120 gallon tank of <br /> waste antifreeze were found on site that has not been reported for 2108, 2019, and 2020. Any material that meets <br /> or exceeds the reportable quantity shall be reported in the facility's business plan.
