The following is an itemized list of hazardous materials violations that have not been <br /> addressed for DUQUE BROTHERS INC as of July 06, 2020. <br /> Open violations from May 21, 2020 inspection <br /> Violation#6 - Failed to complete and/or submit a site map with all required content. <br /> A site map was not completed or submitted as part of the business plan for 2018, 2019, and 2020. A site map shall <br /> contain a north orientation, loading areas, internal roads, adjacent streets, storm and sewer drains, access and exit <br /> points, emergency shutoffs, evacuation staging areas, hazardous material handling and storage areas, and <br /> emergency response equipment. If a site map element is not applicable for your facility then list it on the map and <br /> label as "NA". Immediately log into the California Environmental Reporting System (CERS) at <br /> http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. <br /> ❑ This violation was corrected ❑ This violation will be corrected by (date): <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation: <br /> Violation #7 - Failed to establish and/or submit emergency response procedures for a release or threatened <br /> release. <br /> The emergency response procedures have not been submitted for 2108, 2019, and 2020. A business plan shall <br /> include the following emergency response procedures for a release or threatened release of hazardous materials, <br /> including, but not limited to, the following: <br /> - Immediate notification of local emergency personnel and the Environmental Health Department(EHD) <br /> - Procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to <br /> persons, property, or the environment <br /> - Evacuation plans and procedures, including immediate notice, for the site <br /> Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, upload the <br /> correct or updated information, and submit to the EHD for approval. <br /> ❑ This violation was corrected ❑ This violation will be corrected by (date): <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation: <br /> Violation #8 - Failed to submit a training program or program is not reasonable or adequate for facility. <br /> A training program has not been submitted 2018, 2019, and 2020. Training programs for all employees and annual <br /> training, including refresher courses, shall include familiarity with the facility's Emergency response plans and <br /> procedures in the event of a release or threatened release of a hazardous material. Training shall cover, but not <br /> limited to, all of the following: <br /> (A) Immediate notification contacts to the appropriate local emergency response personnel and to the unified <br /> program agency. <br /> (B) Procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to <br /> persons, property, or the environment <br /> (C) Evacuation plans and procedures, including immediate notice, for the business site. <br /> These training programs may take into consideration of each employee. Immediately prepare or update the facility's <br /> training program, log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, <br /> upload the correct or updated information, and submit to the EHD for approval. <br /> Page 2 of 4 <br />
