L..i *-go, <br /> U-Haul Store No. 70950 Page 2 of 2 <br /> 2701 North Wilson Way October 24, 2014 <br /> Stockton, CA 95215 <br /> • All Investigation Derived Waste (IDW); including all soil cuttings, groundwater, and water <br /> generated from well destruction activities is required to be properly containerized, labeled, <br /> stored and disposed. <br /> • A well destruction report describing all well destruction field activities and including the <br /> signed manifests of the disposal of IDW must be submitted to EHD. The EHD will issue a <br /> "No Further Action" closure letter once and all wells have been property destroyed and the <br /> signed waste manifests have been submitted. <br /> • To proceed with obtaining the destruction permit for these wells the following forms, <br /> documents, and fees must be submitted to EHD at the attention of Angelica Marin: <br /> • A completed San Joaquin County Well and Boring Permit Application form that <br /> describes the destruction of the wells as outlined above along with a site map with <br /> the location of all wells approved to be destroyed and a current well construction <br /> table including borehole diameters, total depths, screened intervals, and the method <br /> of destruction for each well; <br /> • A completed and signed Master File Record form that indicates who the billing party <br /> will be; <br /> • An encroachment permit issued by the City of Stockton for wells MW-7C and MW- <br /> 12B/C that were constructed within the public right-of-way of North Wilson Way and <br /> Sanguinetti Lane; <br /> • The permit fee of$9,100.00 (calculated for 72 monitoring well destructions). <br /> Once the permit has been issued, the EHD requires at least five working days <br /> notification for scheduling the inspection appointments. <br /> This EHD approval is limited to only the methods proposed for the physical destruction of the <br /> above mentioned wells, it is not a concurrence or endorsement for any other conclusions, <br /> conditions or implications contained in the Cardno ATC Consultants Inc, October 2, 2014, Well <br /> Destruction Work Plan. <br /> Please contact Angelica Sandoval Marin (209) 468-2807 or amarin@sjcehd.com if you have any <br /> questions. <br /> Sincerely, <br /> Angelica S.
