0 <br /> • Ensure the Health&Safety Plan(HASP)Approval Form in Attachment 1 is completed and appended <br /> to the HASP, <br /> Obtain for each Field Team Member documentation of that member's formal training required by the <br /> program The Project Manager shall include this documentation in the project file, <br /> • Ensure that Field Team Member receives site-specific training as required by the HASP, <br /> Obtain for each Field Team Member the date that Member's medical surveillance is next due The <br /> Project manager shall note this date in the project file for each Field Team Member If the HASP <br /> requires additional medical monitoring,the Project Manager shall ensure each Field Team Member <br /> receives the additional monitoring <br /> Ensures that the health&safety equipment required by the HASP is available,is in proper working <br /> condition and if required,is properly calibrated prior to use <br /> Required equipment shall include,as appropriate <br /> -monitoring instruments, <br /> -personal protective equipment, <br /> -decontamination equipment and supplies, <br /> -emergency firefighting and first aid equipment, <br /> -sanitation facilities and equipment, <br /> -illumination equipment/portable Iighting,if necessary, <br /> -filed communication equipment, <br /> -site control barricades,tape,stakes,fences(if appropriate),and <br /> -spill control equipment <br /> Appoint at least one qualified individual to serve as Site Health and Safety Officer(SHSO), <br /> Appoint qualified individuals to serve as Field Team Leader(s) The Project Manager may designate <br /> one Field Team Member to serve as both Field Team Leader and SHSO if anticipated site contaminants <br /> can be controlled using Level D or Level C protection, and if the size and scope of field activities <br /> permit one Field Team Member to perform both functions, <br /> Ensure that at least one Field Team Member with current First Aid/CPR training is on-site when field <br /> activities are being performed, <br /> 2 2 SITE HEALTH AND SAFETY_OFFICER(SHSO) <br /> The SHSO shall carry out the responsibilities designated to that positron as specified in the HASP Specifically those <br /> responsibilities include <br /> Ensure health and safety instruments used to determine the Level of Protection are properly calibrated <br /> and used in accordance with applicable calibration procedures, <br /> Ensure methods used to determine Field Team Member exposure to chemical(s)follow the <br /> specifications detailed in the HASP, <br /> Conduct and document periodic inspections and report any deficiencies to the Field Team Leader, <br /> Observe Field Team Members for signs and symptoms of on-site exposure or stress and report <br /> immediately to the Field Team Leader with appropriate recommendations for correction <br />
