• 2 3 FIELD TEAMLEADER The Field Team Leader shall ensure implementation of the HASP and that on-site personnel comply with the HASP SpecificaIIy,the responsibilities include Ensure daily safety meetings are held prior to commencement of the day's field activities and at other times as appropriate, e g,when the HASP is modified,when adverse site conditions develop, Ensure the HASP is modified in writing when the work plan changes,when additional tasks not covered by the HASP will be performed on-site,when site conditions change,or when additional information is available which may impact the health and safety of Field Team Members Direct emergency response efforts required to protect the health and safety of Field Team Members during field emergencies, Perform, or assign the SHSO to perform,a preliminary site evaluation to determine appropriate Field Team Member protection prior to Field Team site entry, Ensure that Field Team Members are provided with a medical examination if they are injured,develop signs of chemical exposure,or are exposed to hazardous substances above exposure limits, I • Ensure that environmental monitoring is performed to identify and quantify Field Team Members' exposure to hazardous substances on-site, Ensure that the site control procedures and practices are implemented, Ensure that illumination and on-site sanitation facilities are used and maintained at the site, Ensure that protective engineering controls,personal protective equipment,and work practices are utilized or implemented on-site to protect Field Team Members, Ensure decontamination procedures are followed on-site to assure that contamination is not spread off- site by Field Team Members or equipment, Ensure that general safe work practices and applicable specific safe work requirements for the drilling and sampling program are followed on-site May assign monitoring and evaluation, observation,training,and/or documentation duties to the SHSO The SHSO function is primarily advisory in nature Unless the Field Team Leader and the SHSO are the same individual,the SHSO should not assume supervisory or enforcement duties, Designate a qualified individual to assume the responsibilities of the SHSO if the SHSO leaves the site when field activities are performed, Be on-site at all times when field activities are performed or shall designate a qualified individual to assume the responsibilities, Notify the Project Manager if Level B protection is required on-site,based on on-site evaluation 2 4 FIELD TEAM MEMBERS All Field Team Members shall comply with the requirements of the program and HASP Further,Field Team Members shall notify the Field Team Leader of SHSO of hazardous or potentially hazardous incidents or working conditions when they anse
