• 2 3 FIELD TEAMLEADER <br /> The Field Team Leader shall ensure implementation of the HASP and that on-site personnel comply with the HASP <br /> SpecificaIIy,the responsibilities include <br /> Ensure daily safety meetings are held prior to commencement of the day's field activities and at other <br /> times as appropriate, e g,when the HASP is modified,when adverse site conditions develop, <br /> Ensure the HASP is modified in writing when the work plan changes,when additional tasks not <br /> covered by the HASP will be performed on-site,when site conditions change,or when additional <br /> information is available which may impact the health and safety of Field Team Members <br /> Direct emergency response efforts required to protect the health and safety of Field Team Members <br /> during field emergencies, <br /> Perform, or assign the SHSO to perform,a preliminary site evaluation to determine appropriate Field <br /> Team Member protection prior to Field Team site entry, <br /> Ensure that Field Team Members are provided with a medical examination if they are injured,develop <br /> signs of chemical exposure,or are exposed to hazardous substances above exposure limits, <br /> I <br /> • Ensure that environmental monitoring is performed to identify and quantify Field Team Members' <br /> exposure to hazardous substances on-site, <br /> Ensure that the site control procedures and practices are implemented, <br /> Ensure that illumination and on-site sanitation facilities are used and maintained at the site, <br /> Ensure that protective engineering controls,personal protective equipment,and work practices are <br /> utilized or implemented on-site to protect Field Team Members, <br /> Ensure decontamination procedures are followed on-site to assure that contamination is not spread off- <br /> site by Field Team Members or equipment, <br /> Ensure that general safe work practices and applicable specific safe work requirements for the drilling <br /> and sampling program are followed on-site <br /> May assign monitoring and evaluation, observation,training,and/or documentation duties to the SHSO <br /> The SHSO function is primarily advisory in nature Unless the Field Team Leader and the SHSO are <br /> the same individual,the SHSO should not assume supervisory or enforcement duties, <br /> Designate a qualified individual to assume the responsibilities of the SHSO if the SHSO leaves the site <br /> when field activities are performed, <br /> Be on-site at all times when field activities are performed or shall designate a qualified individual to <br /> assume the responsibilities, <br /> Notify the Project Manager if Level B protection is required on-site,based on on-site evaluation <br /> 2 4 FIELD TEAM MEMBERS <br /> All Field Team Members shall comply with the requirements of the program and HASP Further,Field Team <br /> Members shall notify the Field Team Leader of SHSO of hazardous or potentially hazardous incidents or working <br /> conditions when they anse <br />
