The San Joaquin County Public Health Services in their letter dated May 19, 1992 concluded that groundwater at the site was contaminated, and that the extent of contamination had not been defined Applied Consultants (AC), San Diego, during May 1993 drilled and installed three additional monitoring wells (MW-4, MW-5, and MW-6) and two soil ' borings (B7 and 138) All six monitoring wells (MW-1 through MW-6) were purged and sampled for various petroleum constituents during May 1993 and results were reported to San Joaquin County Environmental Health Division iSince May 1993 and until September 1994 no activities took place at the referenced site. UHI has retained SOTA to continue with site assessment and ' remediation activities These activities include quarterly monitoring programs to evaluate the quality of groundwater, which has been reported to be impacted at the referenced site Since September 1994, Sol performed six consecutive quarterly groundwater sampling and analysis The last quarterly sampling event was conducted by SOTA in October 1995 and a quarterly report was submitted to San Joaquin County Environmental Health Division on December 15, 1995 /This report was prepared to document the additional site assessment activities which were performed in order to further delineate the lateral extent of groundwater impacted by petroleum hydrocarbons, both "on-site" and `off-site" SOTA's report is prepared per San Joaquin County, Environmental Health Division letter dated August 31, 1994 to UHI and in accordance with Tri- Regional Board Staff Recommendations for Preliminary Evaluation and ' Investigation of UST sites 3.1 Off-site Areas of Environmental Concern ' Potential off-site sources of soil and groundwater impact were investigated by Applied consultants in September 1993 (Applied Consultants, 9-2-93) Aerial photographs and the Polk Directory were also consulted by Applied Consultants for information on the subject site and adjacent properties The results of the limited phase I identified two possible sources that may have impacted the site ' • Harry's Auto Mart, 2662 North Wilson Way, is located approximately 100 feet east, up-gradient, of the subject site County, state and regional lists indicate that 2662 North Wilson Way is permitted for four underground storage tanks One or more of the tanks have leaked ' impacting both the soil and groundwater The site is to be remediated • The property located at 2214 Robindale Avenue, A C Roofing, Incorporated, is located approximately 500 feet east of the subject site A.0 Roofing, Incorporated is permitted to use at least one underground storage tank.
