In a letter dated August 10, 1999 <br /> PHS/EHD requested that work outlined in the Revised Addendum for the Site <br /> Contamination Work Plan dated May 10, 1999 prepared by Upgradient Environmental <br /> Consultants be completed by October 1, 1999. Mr. Ocampo's letter requests that <br /> PHS/EHD allow four more months to complete the work. <br /> Please be advised that in order to meet eligibility requirements for the Underground <br /> Storage Tank(UST) Cleanup Fund a claimant must be or have been the owner or <br /> operator of the UST which is the subject of the claim. Enclosed please find a copy of the <br /> UST Cleanup Fund Guideline and Claim Application Forms. <br /> Work as outlined in the above referenced Work Plan should be completed no later than <br /> February 20, 2000.
