The work plan was approved <br /> with the condition that work be completed within 90 days of the approval date. <br /> The work as outlined in the above referenced work plan has not been completed. The <br /> work as outlined in the above referenced work plan should be completed no later than <br /> October 1. 1999. <br /> In a letter dated March 17, 1999 PHS/EHD directed a well survey to be completed <br /> (including a survey of on site conduits). This work has not been completed. Complete a <br /> well survev as referenced in the above letter and submit the report to PHS/EHD by <br /> October 1. 1999. <br /> A well permit application and $89.00 permit fee should be submitted to PHS/EHD as <br /> soon as possible. A permit application is enclosed. After the permit has been issued the <br /> work may be scheduled.
