Based on the types of violations observed,the facility employees are not trained on proper hazardous waste(HW) handling or the training provided has been inadequate. -The following items were observed not containerized on a shelf in the Exchanges& Returns area of the facility (see photo IMG 3878): -A Wet Platinum Silicone Lubricant Serum labeled"ST for State Regulated Waste and dated 1/29/19(see photo IMG 3879). -A Party String Mul Grafix Can labeled"FL"for Flammable,Aersol/Toxic, Ignitable and dated 1/29/19(IMG 2880). -A Sensodyne ProNamel Mint toothpaste labeled"ST and dated 1/29/19(IMG 3881). -A package of two Dove soap bars labeled"ST and dated 1/29/19(IMG 3882). -A package of two Little Remedies Infant Acetaminophen labeled"ST"and dated 1/29/19(IMG 3883-3884). According to Danielle Heuston, Executive Team Leader,these items should have been placed in a closed tote labeled ESIM, which would then be taken to the hazardous waste storage area of the facility by the end of the day so that the items may be sorted into their appropriate bins.According to Danielle and Thomas Hunter, also an Executive Team Leader,the ESIM bin for the Exchanges& Returns area was taken to the hazardous waste storage area of the facility and had not been returned. -A lip gloss bagged and closed with a zip tie, labeled"ST and dated 1/19/19,was found on the floor beneath a shelf in the Exchanges& Returns area of the facility(IMG 3890). -The following was found in the hazardous waste storage area in the back of the facility: -An open container of Tide laundry detergent was found bagged, labeled"ST,"and dated 1/16/19 in an open unlabeled tote(IMG 3897-3900). -A bottle of Head &Shoulders shampoo was found missing the cap, inside an open unlabeled cardboard box which was inside an open unlabeled tote.The open shampoo bottle was next to three more capped bottles the shampoo,which appeared to have spillage on them from the open bottle of shampoo(IMG 3901).The spillage was also found on a cardboard box containing three containers of laundry detergent capsules(IMG 3902).Thomas Hunter, an Executive Team Leader, could not verify how long the spillage was sitting there, and stated he would have an employee clean-up the spillage and open bottle of shampoo and manage them as hazardous waste. -Eight bottles of Yellow Tail Cabernet Sauvignon were found not containerized, on a shelf in the hazardous waste storage area, and individually labeled"ST and dated 1/10/19(IMG 3924-3925). -Ten bottles of Barefoot Pink Moscato were found in an unlabeled open cardboard box, and individually labeled "ST and dated 1/7/19(IMG 3927-3930). -An unlabeled tote was found in the hazardous waste storage area and contained several items with labels indicating the items were hazardous waste, including the following(IMG 3917-3918): -A Miss Jessies Pill Soft Curls leaking, bagged, and labeled"ST and dated 1/27/19(IMG 3919). -A box of Up& Up Nicotine Gum labeled"FL"and dated 1/27/19(IMG 3921). -A box of Airborne Original Effervescent Tablets labeled"FL"and dated 1/27/19(IMG 3922). -Another unlabeled tote was found in the hazardous waste storage area and contained several items with labels indicating the items were hazardous waste, including the following(IMG 3934-3935, 3947): -A Garnier Medium Brown haircolor labeled"ST and dated 1/28/19(IMG 3936). -A Downy Clean Breeze dryer sheets labeled"ST(IMG 3937). -An Up& Up 100%acetone nail polish remover bagged and closed with a zip tie(IMG 3938). -A box of Quest Protein Cookies labeled"Donate"and dated 1/28/19(IMG 3939-3940). -A box of Wine Cube Rose labeled"ST"and dated 1/28/19(IMG 3941-3942).
