Facility personnel <br /> were unaware of this requirement. <br /> REGULATION GUIDANCE: Hazardous waste generators are required to send a copy of the generator signed <br /> UHWM to DTSC by mail. The only exception is when an UHWM starts and finishes as an electronic manifest in the <br /> e-Manifest System. The generator shall within 30 days of each shipment of hazardous waste submit to DTSC a <br /> legible copy of each manifest used. The mailing address is: DTSC Generator Manifests, Department of Toxic <br /> Substances Control, P.O. Box 400, Sacramento, CA 95812-0400. <br /> CORRECTIVE ACTION: Immediately mail a copy of uniform manifests for the past 3 years and ensure that copies <br /> of all generator signed UHWMs are sent to DTSC within 30 days of shipping the hazardous waste. Also provide a <br /> corrective action statement and supporting documentation to the San Joaquin County Environmental Health <br /> Department(EHD). <br /> This is a Class II violation. <br /> 110 CCR 66262.40(a) Failed to keep signed copy of manifests from the designated facility for three years. <br /> OBSERVATION: Designated facility signed copies of Uniform Hazardous Waste Manifest(UHWM) numbers <br /> 003434384GBF from 7/31/19, 006148471GBF from 5/15/19, 006108479GBF from 2/6/19, 006046384GBF from <br /> 10/25/18 003732969GBF from 7/10/18, 003639475GBF from 2/12/18, 003038632GBF from 2/10/17, and <br /> 003038602GBF from 12/6/16 were not found on-site. Additionally UHWM number 003732299GBF was not found <br /> on site. <br /> REGULATION GUIDANCE: A hazardous waste generator shall keep a copy of each manifest signed in accordance <br /> with section 66262.23(a) for three years or until the generator receives a signed copy from the designated facility <br /> which received the waste. This signed copy shall be retained as a record for at least three years from the date the <br /> waste was accepted by the initial transporter. The hazardous waste generator must submit a copy of the manifest <br /> to the California Department of Toxic Substances Control (DTSC)within 60 days if the destination facility signed <br /> manifest copy is not received from the transporter or designated facility. <br /> CORRECTIVE ACTION: Locate a copy of the destination facility signed manifests and send a copy to the San <br /> Joaquin County Environmental Health Department(EHD). If the destination facility signed manifests cannot be <br /> found then send to DTSC a copy of each generator signed manifest and a statement that the designated facility <br /> signed copy was not received by the generator.
