This uniform manifest <br />Unread <br />includes the 200 gallons of used oil picked up at your facility as <br />Starred <br />well as the oil picked up at other facilities. Although there is a <br />People <br />statement of waste reduction on this manifest, it is signed by <br />Social <br />World Oil Environmental Services, and not you, the generator of <br />Shopping <br />the 200 gallons of used oil. If you cannot obtain a consolidated <br />Travel <br />manifest (similar to the other manifest receipts you have on file <br />Finance <br />for other pick-ups), you have the option of correcting this <br />violation by submitting a statement of action you will take in the <br />Folders <br />future with regards to ensuring that the manifest signed by you, <br />Junk <br />the generator, contains all required information. <br />Pictures <br />1) The name, address, identification number, contact <br />person, and telephone number of the generator, and the <br />signature ofthe generator or the generator's representative. <br />2) The date of the shipment; <br />3) The manifest number; <br />4) The volume or quantity of each waste stream <br />received, its California and RCRA waste codes, the waste <br />stream type listed in subdivision (c), and its proper shipping <br />description, including the hazardous class and United <br />Nations/North America (UN/NA) identification number, if <br />applicable; <br />5) The name, address, and identification number of the <br />authorized facility to which the hazardous waste will be <br />transported; - The transporter's name, address, and <br />identification number; <br />6) The driver's signature; <br />7) A statement, signed by the generator, certifying that <br />the generator has established a program to reduce the <br />volume or quantity and toxicity of the hazardous waste to <br />the degree, as determined by the generator, to be <br />economically practicable. <br />The CaIAG training record provided includes Spill Response <br />Awareness and Emergency Procedures and is sufficient to fulfill <br />the training record requirement.
