The manifest that you sent me is the transporter's (World <br />Oil Environmental Services) uniform manifest. This uniform manifest includes the 200 gallons of used oil picked up at <br />your facility as well as the oil picked up at other facilities. Although there is a statement of waste reduction on this <br />manifest, it is signed by World Oil Environmental Services, and not you, the generator of the 200 gallons of used oil. If <br />you cannot obtain a consolidated manifest (similar to the other manifest receipts you have on file for other pick-ups), <br />you have the option of correcting this violation by submitting a statement of action you will take in the future with <br />regards to ensuring that the manifest signed by you, the generator, contains all required information. <br />1) The name, address, identification number, contact person, and telephone number of the generator, and the <br />signature of the generator or the generator's representative. <br />2) The date of the shipment; <br />3) The manifest number; <br />4) The volume or quantity of each waste stream received, its California and RCRA waste codes, the waste <br />stream type listed in subdivision (c), and its proper shipping description, including the hazardous class and <br />United Nations/North America (UN/NA) identification number, if applicable; <br />5) The name, address, and identification number of the authorized facility to which the hazardous waste will be <br />transported; -The transporter's name, address, and identification number; <br />6) The driver's signature; <br />7) A statement, signed by the generator, certifying that the generator has established a program to reduce the <br />volume or quantity and toxicity of the hazardous waste to the degree, as determined by the generator, to be <br />economically practicable. <br />The CaIAG training record provided includes Spill Response Awareness and Emergency Procedures and is sufficient to <br />fulfill the training record requirement.
