Washington Blvd, Stockton - Riverside Cement Co. <br /> PROPOSAL TO DESTROY 2" MONITORING WELL AT ABOVE SITE <br /> On November 38, 1994, one ( 1) groundwater monitoring well was <br /> Placed on the above property for the purpose of determining if <br /> groundwater had been impacted due a UST which had been removed. <br /> Contaminated soil had previously been excavated and transported <br /> to an approved facility. <br /> Laboratory analyzes of soil and groundwater samples taken from <br /> the well revealed that all tested substances were below detection <br /> limits . Correspondence from your office, dated March 20, 1995, <br /> stated that no further work was required at the site, and a <br /> closure letter would be issued after destruction of the well . A <br /> brief outline of the proposed work is given below; an application <br /> for permit to destroy the well, with the required fee of $60 . 00, <br /> is attached. <br /> PROPOSED WORKPLAN: <br /> 1 . Mobilize V & W Drilling Co. auger rig equipped with 8” OD or <br /> larger clean flights. <br /> 2. Remove Christy box, and drill up PVC casing, cement grout, <br /> and sand pack. Ream the hole several times to insure that all <br /> material has been removed. <br /> 3 . Using a flexible hose or tremie pipe, fill the hole with a <br /> cement/bentonite slurry, making sure there are no voids or <br /> bridges. After augers are removed, allow cement to set for 10-15 <br /> minutes . Add additional slurry if necessary to fill to ground <br /> level . <br /> 4 . Removed materials can be disposed of on site. <br /> Above work is to be performed under the supervision of, and as <br /> directed by, a representative of the San Joaquin County PHS, <br /> Environmental Health Division. That office will be given a <br /> minimum of 48 hours notice prior to doing the work. <br /> 1 <br />
