The following is an itemized list of underground storage tank violations that have not been addressed for LODI OIL INC ARCO #83680 as of May 06 , 2020 . Open violations from May 09 , 2019 inspection Violation #115 - " Designated Underground Storage Tank Operator Identification Form" not submitted within 30 days . Brian Roth is performing designated operator inspections and is not listed on the submitted statement of designated operators in CERS . Notification of the new designated operator was not provided to the EHD within 30 days of the change . Any changes shall be reported to the EHD within 30 days .
