The following is an itemized list of underground storage tank violations that have not been addressed for LODI OIL INC ARCO # 83680 as of May 06 , 2020 . Open violations from May 09, 2019 inspection Violation #201 - Failure to maintain monitoring , maintenance , and follow up action records . Maintenance and monitoring records for the last three years were not found on site . Missing records include work orders and alarm logs addressing the L2 (UDC3/4) , L5 ( UDC 9/10 , and L6 (UDC 11 /12) fuel alarms on 1 /6/19 . These records shall be maintained on site for at least three years . Monitoring records include : ( 1 ) date and time of all monitoring or sampling ; (2) monitoring equipment calibration and maintenance records ; (3) results of any visual observations ; (4) results of sample analysis performed a lab or in the field ; (5 ) logs of all readings of gauges or other monitoring equipment , ground water elevations , or other test results ; (6) results of inventory readings and reconciliations . Immediately locate and maintain all missing maintenance and monitoring records for the last three years on site and submit copies to the EHD . ❑ This violation was corrected ❑ This violation will be corrected by ( date) : ❑ Supporting documents included Describe actions taken or will be taken to correct violation : Violation #202 - Failure to maintain the " Facility Employee Training Certificate" on-site ( after October 1 , 2018 ) . -The training records prior to October 2018 did not include hire dates . -The training records included only single names , not first and last names . All site employees must be trained prior to taking responsibility as a facility employee if after October 1 , 2018 , or within 30 days of hire if prior to October 1 , 2018 . The Facility Employee Training Certificate shall contain accurate information for all employees trained . Prior to October 2018 , the list shall include the dates of training and hire dates for all employees .
