Please complete the <br /> following items within 30 days of the close of escrow[CA Health and Safety Code section 25284 (b) & (c)]: <br /> 1. Create an account and obtain facility access to the facility on the California Environmental Reporting <br /> System (CERS) by contacting the lead user on the account and going to http://cers.calepa.ca.gov. <br /> Ensure that all required uploads are completed in a timely manner. <br /> 2. Underground Storage Tanks — Facility Form — Upload all current information into CERS. Log into <br /> CERS, update the required information, and submit for review by the EHD. Complete a separate tank <br /> form for each tank. (Note: Any change of information must be updated in CERS within 30 days of the <br /> changes). <br /> 3. Underground Storage Tanks—Tank Forms— Upload all current information into CERS. <br /> 4. Certification of Financial Responsibility/Letter from Chief Financial Officer: Complete the form <br /> provided, and make a copy for our records. This document is required to be submitted annually into <br /> CERS. <br /> 3. Underground Storage Tank Monitoring and Response Plans: Upload all current information into <br /> CERS. Retain an approved copy of these plans onsite for future reference. <br /> 4. Report#5021: Update all information for all programs at this site including the UST and Hazardous <br /> Waste programs, sign and date. Bring with all other forms to our accounting office to pay for transfer <br /> fees and the $152/hr UST Consulting fee, if necessary. <br /> 6. If you are the owner, you must submit a notification form identifying the "Designated UST Operator" <br /> for each facility owned. A notification form is enclosed for your use. Upload all current designated <br /> operator information into CERS. <br /> 7. Go to: http://www.waterboards.ca.gov to obtain the current statutes (Underground Storage of <br /> Hazardous Substances) and UST Regulations that you are required to have for your monitoring and <br /> response plan. <br /> 8. Also enclosed for your information is a brochure of Best Management Practices (BMP's) for Retail <br /> Gasoline Outlets that you can use as part of your BMP's for your facility. <br /> 9. Hazardous Materials Management Plan: Contact EHD Hazmat Staff at (209) 468-3420 for <br /> information on updating this plan. <br /> 10. Information and instructions for obtaining a permanent California ID number (enclosed hereto): <br /> Complete the application for a California Hazardous Waste Permanent ID Number and fax it to (916) <br /> 255-4703. <br /> 11. Board of Equalization UST Storage Fee Account Number— Call (916) 322-9669 to obtain a current <br /> number for your facility. <br /> 1868 E.
