An emergency coordinator and modified contingency plan information is lacking.The primary emergency coordinator listed on the contingency plan, observed at the time of inspection, is 3E Company, and the alternate emergency coordinator is Stericycle(see photos IMG 5958-61). It is unknown if 3E Company is available to respond to an emergency by reaching the facility within a short period of time. There must be at least one emergency coordinator on site or on call (i.e., available to respond to an emergency by reaching the facility within a short period of time)with the responsibility for coordinating all emergency response measures. This emergency coordinator shall be thoroughly familiar with all aspects of the facility's contingency plan, all operations and activities at the facility,the location and characteristics of waste handled,the location of all records within the facility, and the facility layout. In addition,this person shall have the authority to commit the resources needed to carry out the contingency plan.Additionally,the following information must be posted by a phone:the name and phone number of the emergency coordinator; location of fire extinguishers, spill control equipment, and if present, fire alarm; and the phone number of the fire department, unless the facility has a direct alarm. Immediately appoint an emergency coordinator that meets the above-mentioned criteria, and post the required information by a phone. A form is provided that can be used for this purpose.
