SAN" J D�l+ �� I 1411" Environmental Health Department <br /> COUNTY�l <br /> Small Quantity Hazardous Waste Generator Inspection Report <br /> Facility Name: Facility Address: Date: <br /> DOLLAR TREE#01260 3728 E HAMMER LN STOCKTON May 06 2019 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 106 CCR 66262.34(d)(2) Failed to train employees on waste handling and emergency procedures. <br /> Based on the types of violations observed,the facility employees are not trained on proper hazardous waste(HW) <br /> handling or the training provided has been inadequate. <br /> The following observations were made at the time of inspection: <br /> -An open cardboard box labeled"Hazmat"containing various items was observed on a black bench next to register 1 <br /> in the store.According to Lacie Geiszler, assistant store manager, the box of items was placed near register 1 on the <br /> previous day and was waiting to be processed and placed in a HW tote, but she did not know who placed the box of <br /> items near register 1.The cardboard box contained the following items(see photos IMG 5869-5904): <br /> o A bag labeled "Hazardous Waste"containing rags or wipes that appeared to be used for cleaning. Lacie stated the <br /> wipes were from a pack of Electronics Wipes.When Lacie scanned the pack of wipes,the hand-held scanner stated <br /> "See guide"for the disposal code. <br /> o Three broken and partially empty 64 fl. oz. containers of LA's Totally Awesome Original Laundry Detergent. The <br /> liquid detergent inside the containers had solidified.The scanner indicated the detergent should be placed in a <br /> State-Regulated, Toxics, Electronics, OTC Pharmaceuticals, Universal Waste(STOU)HW tote for disposal. <br /> o A partially empty 64 fl. oz. container of LA's Totally Awesome Stain Lifter Laundry Detergent.The scanner <br /> indicated the detergent should be placed in a STOU HW tote. <br /> o A single-bag labeled"Hazardous Waste"containing two partially empty 12.6 fl. oz. containers of Palmolive <br /> Essential Clean Original dish soap, and dish soap that leaked out of the containers and into the bag. The bag did not <br /> contain absorbent. The scanner indicated the dish soap should be placed in a STOU HW tote.According to the Dollar <br /> Tree California Training Guide of Hazardous Waste Handling and Processing Procedures, absorbent is to be added to <br /> a bag containing a leaking item to absorb any free liquid, and if a significant amount of liquid is still present in the <br /> container,the container should be double-bagged. <br /> o Four broken prayer candles.The scanner stated"See guide"for the disposal code. <br /> o A bag labeled "Hazardous Waste"and "Windex"containing used paper towels. <br /> o A bag labeled "Hazardous Waste"containing used paper towels. <br /> o A bag labeled "Hazardous Waste"containing an empty bottle of Energy Shot B12 Acai Pomegranate <br /> o An open partially empty 10 fl. oz. container of Bolero Coconut Cream &Vanilla Body Lotion with a missing cap. <br /> o A 3.9 oz. open container of Flarp! Noise Putty,which was missing a majority of the putty.The container still had a <br /> small amount of putty, some of which reached the outside of the container.The scanner indicated the putty should be <br /> placed in a STOU HW tote. <br /> o An open box containing Assured Allergy Diphenhydramine HCl 25 mg capsules.
