San Joaquin County <br /> Environmental Health Department <br /> 1868 East Hazelton Avenue, Stockton, California 95205-6232 <br /> Telephone:(209)468-3420 Fax:(209)468-3433 Web:www.sogov.org/ehd <br /> Small Quantity Hazardous Waste Generator Inspection Report <br /> Facility Name: Facility Address: Date: <br /> Dollar Tree#05403 1 1209 E March Ln, Stockton April 09, 2019 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 106 CCR 66262.34(d)(2) Failed to train employees on waste handling and emergency procedures. <br /> Based on the types of violations observed,the facility employees are not trained on proper hazardous waste(HW) <br /> handling or the training provided has been inadequate. <br /> According to Annalyn Mamaradlo, store manager, all employees are provided HW training, however employees in <br /> management positions are responsible for processing damages using the hand-held scanner,which includes <br /> determining which HW tote an item should be placed in. <br /> The following observations were made in the HW storage area: <br /> -A 64 oz container of LA's Totally Awesome Laundry Detergent was found 1/4 full, bagged with absorbent and <br /> paper towels, marked with a date of 4/9 and initialed "as,"and in a State-Regulated, Toxics, Electronics, OTC <br /> Pharmaceuticals, Universal Waste(STOU)HW tote(IMG 5288-9).When Annalyn Mamaradlo, store manager, <br /> scanned the UPC on the container,the hand-held scanner prompted her to place the detergent into the STOU HW <br /> tote(IMG 5290).When asked what happened to the rest of the liquid that was in the container,Annalyn stated the <br /> detergent spilled and was cleaned up today by Amanda Sodders,freight manager.When asked how she cleaned <br /> up the spill,Amanda stated she used two bags of kitty litter to absorb the liquid, and placed the used kitty litter into <br /> the trash.The used absorbent and the two empty 4.5 Ib cat litter bags were found in a trash can adjacent the HW <br /> storage area (IMG 5291-2).Amanda stated she put some unused kitty litter and paper towels into the bag with the <br /> container to absorb any liquid that may leak from the container.Amanda also stated that she placed the used kitty <br /> litter into the trash, rather than into a HW tote, because employees are told to minimize the amount of items, and in <br /> turn the amount of weight, placed into the HW totes in order to reduce costs.Annalyn and Amanda stated they <br /> would remove the used absorbent from the trash and place it into an STOU HW tote. <br /> -According to page 14 of Dollar Tree's California Training Guide on Hazardous Waste Handling and Processing <br /> Procedures, "as waste is placed in [a hazardous waste tote],the type of waste being added must be checked off the <br /> label."The following HW totes did not have the contents and/or the physical state marked on the label: <br /> -A State-Regulated,Toxics, Electronics, OTC Pharmaceuticals, Universal Waste (STOU) HW tote label,with <br /> accumulation start date 3/29/19,was marked solid for the physical state and did not have the contents marked (IMG <br /> 5288).The tote contained the following: <br /> -A partially filled container of LA's Totally Awesome Laundry Detergent(liquid) bagged with absorbent and paper <br /> towels(IMG 5289). <br /> -A Renuzit Scent Swirls air freshener(IMG 5293). <br /> -A wax prayer candle(IMG 5294). <br /> -Five squishy toys, each containing an electronic light(IMG 5295). <br /> Annalyn placed the liquid detergent into an STOU HW tote that had the physical state marked as liquid. <br /> -An STOU HW tote label, marked with accumulation start date 3/28/19, did not have the contents marked (IMG <br /> 5297).The tote contained the following: <br /> -An empty container of Xtra Care Hand Sanitizer(IMG 5299). <br /> -An empty container of Lysol Clean and Fresh multi-surface cleaner bagged with a small amount of absorbent <br /> (IMG 5300-1). <br /> -A full container of Assured Freshing Mint Mouthwash (IMG 5303). <br /> -An Oxidizers HW tote label,with accumulation start date 3/29/19, did not have the physical state nor the contents <br /> marked (IMG 5304).The tote contained the following: <br /> -a full container of Mister Plumber Drain Opener(IMG 5305). <br /> FA0022071 PR0538317 SCO01 04/09/2019 <br /> EHD 22-01 Rev.09/22/16 Page 6 of 12 Small Quantity Hazardous Waste Generator OIR <br />
