However, according to a <br /> color-coded document titled "Products to be Treated as Hazardous Waste"found in the HW storage area, all hand <br /> sanitizers are to be placed in the Aerosols, Flammables, and Combustibles(AF) HW tote(IMG 5320).Annalyn <br /> placed the hand sanitizer into an AF HW tote. <br /> -A full container of Assured Freshing Mint Mouthwash was found in an STOU HW tote(IMG 5297, 5303). However, <br /> according to the"Products to be Treated as Hazardous Waste"document, mouth wash is to be placed in the AF <br /> HW tote(IMG 5321).Annalyn placed the mouth wash into an AF HW tote. <br /> -A Dollar Tree basket containing 38 bottles of Nature's Measure Glucosamine Joint Therapy tablets, 31 of which <br /> had a best buy date of 7/7/2018 and 7 of which had a best buy date of March 2019,was found in an area of the <br /> stock room located behind the hazardous waste storage area (see IMG 5306-8, 5311, 5313, 5317).The bottle <br /> labels provided Supplemental Facts.According to Annalyn Mamaradlo, store manager, she suspected an employee <br /> removed the bottles from the sales shelf since they were expired, but did not determine what to do with the bottles. <br /> According to page 13 of Dollar Tree's California Training Guide on Hazardous Waste Handling and Processing <br /> Procedures, "any waste product whose label provides"Drug Facts"or"Supplemental Facts"must be managed as <br /> STOU, regardless of the product's ingredients."Annalyn placed the bottles into two hazardous waste bags and into <br /> an STOU HW tote(IMG 5316, 5318). <br /> -An empty container of LA's Totally Awesome Bleach was found on the ground adjacent the facility's mop sink. <br /> Annalyn scanned the barcode on the bleach and the hand-held scanner prompted her to place the item into the <br /> Oxidizers(OX)HW tote(IMG 5327-30).Annalyn placed the empty bleach container in a bag and into the OX HW <br /> tote. <br /> -A Dollar Tree Potentially Hazardous Material Management poster was found posted on the wall behind the HW <br /> storage area, but was blocked by store supplies on the top shelf of the HW storage area(IMG 5322-3). <br /> -When asked how employees determine which HW tote an item should be placed in when the hand-held scanner, <br /> the HW labels, and the"Products to be Treated as Hazardous Waste"document do not provide a HW <br /> determination for the item,Annalyn was not familiar with the Potentially Hazardous Material Management poster <br /> posted behind the HW storage area, nor was she familiar with the phone number listed on the poster to call in case <br /> questions came up.Annalyn was also not familiar with how to access product Material Safety Data Sheets (MSDS) <br /> electronically through Dollar Tree's computer portal, nor by calling Dollar Tree's MSDS vendor, as indicated on page <br /> 11 of Dollar Tree's California Training Guide on Hazardous Waste Handling and Processing Procedures. <br /> The generator must ensure that all employees who handle hazardous waste are thoroughly familiar with proper <br /> waste handling and emergency procedures. <br /> Provide proof of re-training and the training materials to the EHD for employees whose responsibilities include <br /> hazardous waste. <br /> This is a Class II violation. <br /> FA0022071 PR0538317 SCO01 04/09/2019 <br /> EHD 22-01 Rev.09/22/16 Page 7 of 12 Small Quantity Hazardous Waste Generator OIR <br />
