SAN J O A Q U I N Environmental Health Department <br /> AMENDED <br /> 9/24/2019 <br /> Large Quantity Hazardous Waste Generator Inspection Report <br /> Facility Name: Facility Address: Date: <br /> ALEXANDERS DIESEL REPAIR 1055 S STOCKTON ST, STOCKTON September 19, 2019 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> (3)a written description, including a syllabus and/or outline,of the type and amount of both introductory and <br /> continuing training that will be given to each person filling a position listed under paragraph(d)(1)of this section; <br /> (4)employee-signed or-certified records that document that the training required under subsections(a), (b),and (c) <br /> of this section has been given to,and completed by, each employee. <br /> (e)The owner or operator shall maintain training records on current personnel until closure of the facility and training <br /> records on former employees for at least three years from the date the employee last worked at the facility. <br /> Personnel training records may accompany personnel transferred within the same company. <br /> CORRECTIVE ACTION: Immediately receive training and provide training documentation to the San Joaquin <br /> County Environmental Health Department(EHD)within 30 days. The documentation must include a list of training <br /> topics,date of training,job title, name of employee,written job description(qualifications and duties),and employee <br /> signed (or certified)record that documents the training. <br /> This is a repeat violation, Class II. <br /> 118 HSC 25160.2(b)(3)Failed to keep copies of consolidated manifesting receipts for three years. <br /> OBSERVATION: The last oily solids(absorbent,oil filters)disposal record available for review was dated 5/3/2019. <br /> No filter waste was observed on site. No used oil records were observed on site from 10/12/2018-1/4/2019. <br /> According to Alexander Dunn, Owner,used oil was generated and picked up by a disposal company in that time <br /> frame. <br /> REGULATION GUIDANCE: The generator shall retain each consolidated manifest receipt for at least three years. <br /> This period of retention is extended automatically during the course of any unresolved enforcement action regarding <br /> the regulated activity or as requested by the department or a certified unified program agency. <br /> CORRECTIVE ACTION: Immediately locate a copy of the missing consolidated manifest receipts specified above <br /> and maintain them on site.
