You were directed to submit to the EHD a well destruction permit application along <br /> with the required $60.00 permit fee and $315.00 permit processing and inspection fee for <br /> each parcel of land on which wells to be destroyed are located and any applicable access <br /> agreements or encroachment permits. No permits or fees have been submitted to this <br /> agency. Please proceed with submittal of the permits and with scheduling the destruction of <br /> all groundwater monitoring and remediation wells. Deadline for completion of the well <br /> destructions is January 30, 2009. Manifests for removal of all wastes associated with the site <br /> investigations are due within 30 days following completion of the well destructions. <br /> You were also directed to provide to the EHD certification in writing that all record fee title <br /> owners have been notified of your request for closure pursuant to section 25297.15(a) of the <br /> Health and Safety Code. If you are the sole property owner, please provide a statement of <br /> that fact to the EHD (form enclosed). <br /> Forty eight hours notice to the EHD is required prior to all site fieldwork.
