To proceed with closu le of this site, please submit to the EHD <br /> a well destruction permit application.along with the required $60.00 permit fee and $315.00 <br /> permit processing and inspection fee for each parcel of land on which wells to be destroyed <br /> are located and any applicable access agreements or encroachment permits. <br /> Please provide to the EHD certification in writing that all record fee title owners have been <br /> notified of your request for closure pursuant to sectio 125297.15(a) of the Health and Safety <br /> Code. If you are the sole property owner, please provide a statement of that fact to the EHD <br /> (form enclosed). <br /> Forty eight hours notice to the EHD is required prior to all site fieldwork.
